CHD LIFE+ Family Information
CHD LIFE+ is a national study which aims to design effective models of care in each Australian state to support the long-term developmental needs of children with congenital heart disease.
One aspect of this CHD LIFE+ study is the evaluation of the care that children with congenital heart disease receive in Queensland, through a care pathway known as CHD LIFE.
As part of our evaluation, we are consulting consenting families who have had direct experience with the CHD LIFE care pathway in Queensland.
